Please read these Terms and Conditions ("Terms") carefully. By booking, paying for, or receiving any service from CrystaDrip LLC ("CrystaDrip," "we," "us," "our"), you agree to be bound by these Terms in their entirety. If you do not agree, do not book or use our services.

1. Services & Scope

CrystaDrip LLC provides mobile intravenous (IV) hydration and wellness infusion services administered by licensed Registered Nurses (RNs) under physician/FNP oversight. All services are for general wellness purposes only and do not constitute medical treatment, diagnosis, or a substitute for professional medical care. We do not treat, cure, or prevent any disease or medical condition.

2. Eligibility & Age Requirement

You must be 18 years of age or older to receive services. By booking you represent that all information provided is accurate. Minors will not be served under any circumstances.

3. Health Intake & Medical Disclosure

You must complete a health intake form before every session. You are required to disclose all known medical conditions, allergies, medications, supplements, and relevant health history. CrystaDrip reserves the right to refuse or discontinue service at any time if clinically appropriate or if accurate health information has not been provided. Failure to disclose accurate information increases your risk of adverse events and releases CrystaDrip from liability for resulting harm.

4. Assumption of Risk

IV therapy involves needle insertion and infusion of fluids directly into the bloodstream. Inherent risks include but are not limited to: bruising, hematoma, phlebitis, infection, allergic or anaphylactic reaction, vein damage, air embolism, electrolyte imbalance, and cardiac events. By booking, you voluntarily and knowingly assume all risks. You acknowledge that no guarantee of safety or results has been made.

5. No Medical Guarantee; No Diagnosis

CrystaDrip makes no representations regarding the effectiveness, outcomes, or results of any service. Individual results vary. No statement made by CrystaDrip—verbal, written, or digital—constitutes a medical claim, diagnosis, prognosis, or guarantee of any kind. Testimonials are individual experiences and are not typical or guaranteed.

6. Payment & Pricing

All prices are in U.S. dollars. Payment is required at time of booking via secure payment processing. Prices are subject to change. Group discounts (10% for 3+ guests) apply only to single-session bookings. Membership fees are billed monthly on a recurring basis. All sales are final unless a cancellation qualifies under Section 7.

7. Cancellation & Refund Policy

More than 24 hours before appointment: Full refund, no charge.
2–24 hours before appointment: 50% cancellation fee.
Less than 2 hours / No-show: Non-refundable. Full charge applies.
CrystaDrip cancellations: Full refund or complimentary reschedule if we cancel due to staff unavailability or safety concerns.

8. Late Cancellation & No-Show

If our nurse arrives at your location and cannot access you within 15 minutes of the appointment time, the session is a no-show and is non-refundable. A $50 late cancellation fee applies to cancellations within 2 hours of the appointment time.

9. Membership Terms

Memberships are monthly recurring subscriptions. One membership is per individual; memberships are non-transferable. To cancel a membership, written notice must be provided at least 7 days before the next billing date via email to billing@crystadrip.com. Unused sessions do not roll over and are forfeited at the end of each billing cycle. No refunds are issued for partially used membership months.

10. Service Location & Access

Services are provided throughout Manhattan, Brooklyn, Queens, and Staten Island. You are responsible for ensuring the service location is safe, legally accessible, and appropriate for IV therapy. CrystaDrip may decline to service a location at the nurse's sole discretion for any safety-related reason. A travel supplement may apply for locations outside our service areas.

11. Limitation of Liability

To the maximum extent permitted by law, CrystaDrip LLC, its owners, officers, employees, nurses, contractors, and affiliated providers shall not be liable for any direct, indirect, incidental, special, consequential, or punitive damages arising from or related to your use of our services—including adverse reactions, medical complications, injury, or dissatisfaction. Our total liability is capped at the amount paid for the specific session in question.

12. Indemnification

You agree to indemnify and hold harmless CrystaDrip LLC and its affiliates, officers, employees, nurses, and agents from any claims, damages, costs, or expenses (including reasonable legal fees) arising from: (a) your use of services; (b) your breach of these Terms; (c) inaccurate health information you provided; or (d) your violation of any applicable law.

13. HIPAA & Health Information

CrystaDrip complies with applicable HIPAA requirements. Your Protected Health Information (PHI) is used solely to deliver services and is never sold. Disclosure is limited to treating providers, as required by law, or with your explicit written consent.

14. Governing Law & Dispute Resolution

These Terms are governed by the laws of the State of New York. Any dispute not resolved informally within 30 days shall be submitted to binding arbitration in New York County under AAA rules. Class action and jury trial rights are waived to the fullest extent permitted by law.

15. Modifications

CrystaDrip reserves the right to update these Terms at any time. Continued use of services after any modification constitutes acceptance.

This Privacy Policy explains how CrystaDrip LLC collects, uses, stores, and protects your personal and health information when you use our website or services.

1. Information We Collect

Personal Information: Name, phone number, email address, mailing/service address, and payment information collected at booking.

Protected Health Information (PHI): Health intake form responses, medical history, allergies, medications, and any health-related disclosures. This is treated as PHI under HIPAA.

Technical Data: IP address, browser type, device type, and session data collected automatically via cookies and analytics.

2. How We Use Your Information

We use your information to: schedule and deliver IV therapy services; process payments; send appointment confirmations and reminders; communicate regarding your care; comply with legal obligations; improve our services; and send promotional communications (with your consent, which you may withdraw at any time).

3. HIPAA Compliance

CrystaDrip operates in compliance with the Health Insurance Portability and Accountability Act (HIPAA). Your PHI is used exclusively to provide care and will not be sold, rented, or shared with third parties for marketing. PHI may be shared with: our supervising medical providers for care coordination; third parties as required by law; or parties you have explicitly authorized in writing.

4. Third-Party Processors

We work with vetted third parties who process data on our behalf under confidentiality obligations:

Stripe — PCI-DSS compliant payment processing. We do not store your full card number.
Netlify Forms — Booking form submission delivery.
Netlify — Website hosting and serverless functions.
Anthropic — AI wellness advisor feature (no PHI transmitted).

5. Cookies

We use cookies to enhance your browsing experience and remember your preferences. You may decline non-essential cookies. Essential cookies are required for basic site function and cannot be disabled. We do not sell your data to advertisers or use third-party advertising cookies.

6. Data Retention

Personal data is retained as long as necessary to fulfill the purposes described here and to comply with applicable law. Health intake records are retained a minimum of 7 years per New York State requirements. You may request deletion of non-PHI personal data by contacting us.

7. Your Rights

You have the right to: access your personal data; correct inaccuracies; request deletion (subject to legal retention requirements); opt out of marketing at any time; and lodge a complaint with a regulatory authority. Submit requests to privacy@crystadrip.com.

8. Data Security

We use industry-standard technical and organizational safeguards including SSL/TLS encryption, secure payment processing, and access controls. No system is 100% secure; we cannot guarantee absolute security of data transmitted over the internet.

9. Children

Our services and website are not directed to persons under 18. We do not knowingly collect data from minors.

10. Changes to This Policy

We may update this policy at any time. Changes will be posted here with a revised effective date. Continued use of our services constitutes acceptance.
